Waiver

Because physical exercise can be strenuous and subject to risk of serious injury, Chris Sprague urges you to obtain a physical examination from a doctor before using any exercise equipment or participating in any physical activities. You agree that it is solely your responsibility to consult with a physician prior to commencing any exercise or fitness program, to remain under medical supervision if indicated, or to see medical assistance in the event of any injury, and inform Chris Sprague of any change in your medical condition. You (and any participating family member or guest) agree that if you engage in any physical exercise, you do so at your own risk. This waiver of release of liability includes without limitation, all injuries which may occur as a result of: (a) your use of any and all equipment and your participation in any activity, class or program, or personal training instruction. (b) the sudden unforeseen malfunctioning of any equipment (c) slipping and falling while working with Chris Sprague.

(e) your personal items are not held responsible by Chris Sprague. You acknowledge that you have read this release carefully and understand it is a release of liability. You expressly agree to release and discharge Chris Sprague and all affiliates, employees, representatives, or assignees from any and all claims or causes of actin and you agree to voluntarily give up or waive any right that you may otherwise have to bring legal action against Chris Sprague for personal injury or property damage. To the extent that statute or case law does not prohibit release for negligence, this release is also for negligence on the part of Chris Sprague, and his employees, agents, or representatives. If any portion of this release of liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release shall remind in full force and effect and the offending provision or provisions severed here from. A 24-hour cancellation policy is strictly enforced. (One) session fee will be charged for late cancellations.
